According to Immigrations and Customs Enforcement (ICE) all international students studying in the states with an F-1 visa should communicate with their Designated School Official (DSO) prior to traveling outside of the US. The purpose of this form is to comply with this requirement. Please fill in this document to the best of your ability and notify the DSO immediately if travel plans are extended or changed in any way.
